Completing the Application Form
- The Application form includes a 2-page instruction sheet. Please read these instructions carefully; failure to properly complete the application form will require the form to be returned to you and will result in delays in processing your application.
- Make sure you answer EVERY question. If something is not applicable, put N/A in the box. Provide ALL information about arrests, convictions, dismissals, etc. List ANY alcohol related offense you have EVER been charged or convicted of at any time in your life. Failure to make full disclosure will result in denial of your application.
- If you are applying for a Class 1 license, make sure you also sign and date the Authorization to Disclose Health Information form (last page stapled to the application packet). If you are applying for a Class 2 license, you only need to sign and date the Authorization form if you answer “yes” to one of the relevant questions (see the instructions on the application form).
- MAKE SURE YOU WRITE IN THE REASON YOU ARE APPLYING FOR THE LICENSE. Failure to do so will result in your application being returned to you and delays in processing the application.
- Sign and date the form – and make sure the test administrator completed, signed and dated the form.
- The LAW ENFORCEMENT AGENCY is responsible for submitting applications to the BCI. DO NOT submit it yourself. See SUBMITTING THE APPLICATION FORM for more information.
