Obtaining the Application Form
- ND residents: The application form is available from your local chief of police or sheriff, or from the civilian certified test administrators.
- Out of state residents may send a written request for an application form to the BCI, Attention CWL, PO Box 1054, Bismarck, ND 58502. Out of state residents should bear in mind that any testing required for renewal must be conducted and completed within the state of North Dakota.
